GP PATIENT SURVEY
Some patients registered at this GP practice will soon be invited to take part in a national survey about their experiences of local NHS services. It covers issues that concern patients, such as access to care and satisfaction with treatment. If you receive a questionnaire, please help NHS England by filling it in as soon as possible. It is really important that we hear your views, even if you have received a questionnaire like this before. Your doctor may suggest you make an appointment for one of the following clinics:
The nurses carry out the full range of nursing procedures including immunisations, blood tests, dressings and cervical smears. They also take a major part in our health promotional clinics.
Ante-natal and Booking Clinics (Community Midwife) by appointment only, arranged by the midwife.
Child Immunisations (Nurse, doctor available) Tuesday 09:00 - 18:00.
By appointment only, sent from the Child Health Department. If your child has not been sent an appointment, please contact the surgery to make a routine nurse appointment.
8 Week Development Check/Post-natal (Doctor) by appointment only, arranged and sent out by our admin team.
They are now based off site and require a referral to their services. This can be made at the request of the GP.

If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.
We are able to provide a full range of travel vaccinations and the surgery is a designated Yellow Fever Centre. Immunisations are given by the nurse/GP. Some vaccinations need to be done as part of a course, so it is helpful to plan ahead.
You do not need to be a registered patient at the Surgery to access the Yellow Fever Clinic. If you are planning to travel abroad, please complete a travel health questionnaire which is available to download or collected from reception.

It is important to make this initial appointment as early as possible - at least 6 weeks before you travel. Some vaccines have to be ordered as they are not a stock vaccine.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
