LGV/HGV/Taxi Medical renewal – At least 6 weeks left before it is due from the request date
All medical reports/forms completed need to have a copy taken
All records requests will be completed within 28 days
We have no specfic time frame that we must complete medical reports/form in
Holiday cancellation – Patient must have been seen by GP, or we need to have received correspondence from hospital/clinic for GP to provide the supporting information. We do NOT provide TWIMC for holiday cancellations they need to provide the form from their insurance company.
We do not complete any forms or provide specific information for DWP regarding ANY benefits. If patient needs any supporting information, then they will need to complete a records request form
We do not do the following:
- Passport Verification
- Power of attorney
- Proof of Address
- Shotgun/firearm unless it is for occupational purposes
- Home office letters
- Housing letters to the council
- Any letters or supporting information for DWP
- Fit to fly
- Blue Badge
